Your new role
As a Chronic Disease Management Health & Wellbeing Nurse your main duties and responsibilities would include preparing GP Management Plans with patients on behalf of the GP, preparing Team Care Arrangements often alongside the GPMP and conducting health assessments to assist in early identification of chronic disease. You must hold an AHPRA registration as an Enrolled or Registered Nurse. A successful candidate would also hold the following attributes:
